Dalton Beeler lacked a hazardous materials endorsement, required placards, shipping papers, and emergency response information while moving the cargo from South Pittsburg to Knox County. Tennessee Highway Patrol's post-incident inspection documented multiple federal violations, with findings forwarded to regulators. A brake fire sparked the incident — the most predictable failure mode in trucking meeting the most dangerous cargo transported. Industry data reveals the Ooltewah incident was not isolated. The worst-performing fireworks carriers accumulated more than 1,400 brake violations, with 334 resulting in out-of-service orders.
