A Boeing 767-300 freighter operated by 21 Air overran runway 30 at Miami on September 6, striking ground vehicles and killing five people. National Transportation Safety Board analysis of cockpit recordings reveals the sequence of events leading to this accident.
One pilot repeatedly cautioned about excessive speed during final descent, with minimal response from the flight deck. The aircraft generated multiple terrain-proximity and sink-rate warnings between 1,000 feet and touchdown.
Four separate 'too low, terrain' alerts sounded after the 100-foot callout. One pilot requested a go-around approximately 5 seconds before the freighter departed the runway surface. Flight-data recorders indicate throttles briefly advanced to go-around power before reducing to idle.
The Amazon Air-branded cargo flight touched down before overrunning the runway.
